This form can be printed on your printer and filled out by simply placing the cursor above the form in the empty space and dragging it as far as it will go to the bottom. The area to be printed should now be highlighted. Then go to File...Print (drop-down menu) at the top of your browser and click "Selection" to print ONLY the form. If you do not choose "Selection" you will end up printing the entire POD Alert. When you choose "Selection" you will print only the area highlighted. Fill it out and bring in with you on Sunday; then give it to Sue or one of the board members.
